Caregivers are encouraged to fill out a Smart Watch Medical Alert Form (PDF) that includes the name and physical description of the person who they are concerned might wander off. They are urged to send a photo so responding officers know exactly what the individual looks like. The information is stored in a departmental database which allows officers to review critical information from their cruiser laptop if there is a disappearance. The information includes a summary such as name, nickname, age, address, height, weight, eye color, hair color, identifying marks, medical conditions, favorite attractions (where the person tends to go) and how to effectively approach the person.

Family members, friends or caregivers can complete the Safe Watch form either electronically or by printing a copy and returning it to the police station so that the most accurate information is available.

This is a free, voluntary, and confidential program that is designed to better equip emergency personnel when responding to a crisis situation. The information is not for general release.
